The Carolinas Opioid Reduction Effort (CORE) program has shown significant success in providing patients with opioid-minimizing recovery options, dramatically reducing the risk of addiction, as well as improving recovery experiences.
The work behind CORE began in 2017 with a pilot study focused on opioid-free pain management in shoulder replacement surgeries. Building on those promising results, OCRI officially launched CORE in March 2018. Since then, OCRI has led the charge in transforming how orthopedic care is approached by offering patients a safe and effective alternative to opioids from the outset of their treatment. Backed by OrthoCarolina physician research, CORE is implementing proactive, prevention-focused strategies that empower patients to make informed decisions about their pain management, providing them with opioid-reducing options.CORE has not only improved individual patient outcomes but is also setting a new standard for the healthcare community at large, positioning it as a national model for reducing opioid use during and after orthopedic surgery.
The success of the program is evidenced by its groundbreaking research, partnered with Atrium Health, Charlotte Surgery Center, Novant Health, and OrthoCarolina, which standardized non-opioid protocols for a year. Published in The Journal of Bone & Joint Surgery, one of the most prestigious peer-reviewed journals in the field, the study confirmed the clinical success of the program. Patients reported less pain, improved comfort, and fewer side effects compared to traditional opioid-based regimens.
Building on this momentum, The Duke Endowment has invested well over $1 million across three grants to help scale the program statewide. OCRI’s next steps include:
